Short-term Rehabilitation
When your loved one requires skilled nursing care, you can trust in the compassionate care offered at Leslie Lakes. We focus on personal and individual attention, because we realize that everyone is unique and have individual medical and personal needs. Leslie Lakes skilled nursing and rehabilitation center provides each resident with individualized and goal-oriented treatment programs. Our rehab team consists of therapists, including speech, occupational and physical. Our focus is to help our residents return home.
Rehabilitation means reshaping lives and restoring lost abilities after surgery, illness or injury. Our approach provides the ideal solution for seniors who need help with daily activities, such as bathing, dressing and medical supervision during their post-hospital stay. While working daily with our full-time therapy staff, our residents enjoy beautifully appointed living spaces, delicious meals, engaging social activities, transportation, personalized assistance with care. Leslie Lakes focuses on preparing our short-term stay residents back to independent living in their home and in the community.
A three day stay in the hospital and a diagnosis of any of the following will qualify you for rehabilitation or our skilled care unit:

Rehabilitation stays usually range from fourteen to one hundred days. An average length of stay is forty-seven days. During this time the patient’s room and board, medications, meals, x-rays, laboratory tests, and physician visits are covered by Medicare and/or supplemental insurance. There are usually no out of pocket expenses.
Your loved one will receive up to three hours of therapy per day and twenty-four hour nursing supervison. The patient’s primary physician will follow them while at Leslie Lakes skilled nursing facility. He or she must be seen once every thirty days. Upon discharge back into the community, our therapists will do a home evaluation to ensure the transition is as smooth as possible. The rehabilitation team will evaluate your home and recommend adaptive equipment and physical modifications to make living at home easier. We also provide referrals to community services to ensure your at-home needs are served.

Long-term Rehabilitation
It is our goal to make sure our long-term care residents feel they are at home and well cared for. We try to make our home as much like their own by providing he or she with many wonderful services and amenities (see listing below).
Residents are encouraged to bring anything that will make them feel more comfortable and fit safely in their room. It is nice to be surrounded with precious items that are memorable to them.
